Feds blame welding company for Back Bay fire that killed two firefighters

WCVB reports OSHA has cited D & J Ironworks for not taking even rudimentary precautions while welding on a windy March day behind one Beacon Street building, which led to a fire in the neighboring building that trapped and killed Lt. Edward Walsh and firefighter Michael Kennedy.
