Pedal misapplication is a common cause of such accidents. So much so that the phrase was coined for it.

Newspaper offices really need to be more careful. Was it even wearing light-colored clothing? Was it even licensed and insured? If you're going to share the road with cars, you just need to pay more attention, whether you're on foot, on bike, or made largely of brick

Who among us has not driven into the front of a building while trying to parallel park just exactly perfectly?
Need different drivers licences
I have always been a big advocate of having separate tiers of Class D drivers licenses. If you suck, like this guy, you should not be allowed to parallel park or drive 1mph over the speed limit. If you have demonstrated advanced driving skills and hand eye coordination and have a clean driving record, you should be allowed to parallel park, make U-turns, and drive over the speed limit. It would incentivize people to drive better IMHO.
Yeah, two types of licenses would be great.
Type #1: You follow traffic laws and are allow to drive.
Type #2: You aren't allowed to drive.
