Shouldn't safety tethers be standard? Aside from welding them shut, what other solutions are being explored? Between this and the O'Neill tunnel incident last year, it should be a no-brainer.
Because many of them have nothing to do with sewers - they cover utility conduits, water mains, the underground lairs of pizza-eating fighting turtles, etc.
