Welding sets construction material on fire in new Fenway building under construction

Boston firefighters responded to 421 Park Dr., now under construction as a 13-story addition to the former Landmark Center, for a fire that started by welding and construction debris on the fourth floor but spread several floors upward, around 5:10 p.m.
The fire was declared out about 25 minutes later.
The ground floor of the building, being developer by Samuels and Associates, is slated to become a new Star Market, to replace the one on the other side of Boylston Street.

How does this happen?
I thought any commercial welding required a firefighter on scene to prevent anything from getting out of hand? Wasn't this the cause of the tragedy ~10 years ago in Boston when two firefighters died going into a building fire that was caused by welding?
Not for major construction
This is a large building under construction, not a renovation of an occupied building as was the case for the Beacon St tragedy.
Still someone's mistake that allowed this fire to start.
